Appointment Request
Please call the office at 508-697-7102 to schedule an appointment. If for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible.
First Visit
Your first appointment will consist of a consultation that will end with us discussing your diagnosis and treatment options, though sometimes treatments can be performed on the same day if possible. However, depending on the complexity of the treatment plan, a second appointment may be needed.
WE ALSO ASK THAT YOU BRING THE FOLLOWING INFORMATION AT THE TIME OF YOUR VISIT:
- Your Patient Medical History, Office Policy, Informed Consent, and HIPAA forms. (Forms can be found on home page and/or under Patient Information-Patient Forms)
- Your dental history and x-rays from any previous dental offices.
- A list of medications you are presently taking
- Your insurance card
All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
Insurance and Billing
We accept most traditional insurance plans, contact our office to verify acceptance of your plan. Fisher Family Dentistry does not participate in Health Management Organizations; however, we will be happy to file your insurance claims for you.
Payment Options
We accept checks, cash or credit cards. We also offer a flexible payment plan. Please see our Financial Coordinator for details. We are happy to file insurance for your reimbursement as long as you are free to choose your own dentist.
As a condition of treatment by this office, payment is due on the date of service. Any major services (ex: post and cores, crowns, prosthetics) financial arrangements must be made in advance. The practice depends upon reimbursement from patients for the costs incurred in their care. Financial responsibility on the part of each patient must be determined before treatment.
All dental services performed without financial arrangements must be paid for in full at the time services are rendered.
Patients with dental insurance understand that they will be responsible for their co-payment and/or deductible. This office will help prepare the patient's insurance forms and/or assist in making collections from your insurance company, and will credit any collections to the patient's account. However, this dental office cannot render services on the assumption that our charges will be paid by an insurance company.
Office Policy
I understand that Fisher Family Dentistry, P.C. has a 24 hour cancellation policy. There will be a charge if an appointment is cancelled without 24 hour notice and /or missed. Fisher Family Dental will do our best to confirm all appointments but it is patients responsibility to remember their schedule appointments.
