Patient Information for Our Office in Bridgewater, MA
First Visit
Your first appointment will consist of a consultation that will end with us discussing your diagnosis and treatment options, though sometimes treatments can be performed on the same day if possible. However, depending on the complexity of the treatment plan, a second appointment may be needed.
WE ALSO ASK THAT YOU BRING THE FOLLOWING INFORMATION AT THE TIME OF YOUR VISIT:
- Your Patient Medical History, Office Policy, Informed Consent, and HIPAA forms. (Forms can be found on home page and/or under Patient Information-Patient Forms)
- Your dental history and x-rays from any previous dental offices.
- A list of medications you are presently taking
- Your insurance card
All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
